JOB DESCRIPTION

 

TITLE:  JOB COACH – APPLIED ABILITIES

Immediate Supervisor:        Program Director and Coordinator

Supervision Exercised:       Persons Served

 

SUMMARY:

Under the supervision of the Director of Applied Abilities and Program Coordinator, this position is responsible for the implementation of Individual Habilitation Plans that will lead to or continue to teach skills that allow the individuals that we serve to become more independent in the community.

 

ESSENTIAL FUNCTIONS:

  • Provides on-the-job training to groups and/or individuals
  • Responsible for providing skill or developmental training in various activities as designated; assists with personal care/hygiene as needed
  • Contributes to positive relations between community members, persons served, vendors and other Agency staff
  • Assists program participants with community integration
  • Assists program participants in goal achievement
  • Organizes social and vocational groups in promoting independence in the community
  • Completes all required documentation in a timely manner
  • Uses crisis intervention techniques in emergency situations when persons served are in danger of injuring themselves or others
  • Ensures work supplies and equipment are in good and safe condition
  • Ensures a safe working environment that meets OSHA and other regulatory agency requirements
  • Complies with all UCP WORK Inc., Tri Counties Regional Center and Community Care Licensing policies and procedures
  • Must be able to work at other UCP WORK, Inc. locations, when needed.
  • Must be able to transport participants, unless waived by the Executive Director.
  • Other related duties, responsibilities and special projects on an as needed basis

PHYSICAL REQUIREMENTS:

  • Ability to stand, walk and sit for long periods of time
  • Ability to go up and down stairs
  • Ability to speak, read, hear and understand technical written materials
  • Lifting may be required; may exceed 25 lbs
  • Ability to bend, lift and carry
  • Full awareness of environmental stimuli
  • Full range of motion above the head
  • Ability to implement Crisis Management Safety & Restraint techniques as needed

QUALIFICATIONS:

  • High school diploma or equivalent required
  • Computer skills (Microsoft word)
  • Experience working with persons with developmental and/or other disabilities
  • Demonstrated ability to maintain confidentiality regarding consumer and other related information
  • Excellent verbal and written communication skills
  • Ability to complete tasks with numerous interruptions
  • Ability to work with little direct supervision
  • Ability to manipulate numbers
  • Computer proficiency
  • Compassion and respect for persons with disabilities
  • Valid CA Driver’s License
  • Valid certification in CPR/1st Aid (Training available)
